At the very onset of the current semester (Semester 2, AY 2013-2014) the Educational Technology Center successfully launched Online Forms for Students to address their various academic concerns. The forms available online are: Appeal, Course Withdrawal, Request for Course Description and Repeat a Course Forms. This project aims at providing an easy and far more convenient way for students to address their various academic concerns. These forms can be accessed by students anytime, anywhere using an Internet Connection. This service will also help the Academic Staff in discharging their duties more efficiently and instantaneously. This project was made possible by our programmer, Mr. Anthony Madlambayan from the IT Department who works hand in hand with the Educational Technology Center. Ms. Marilou Paderes, ETC-CSS Staff, designed the Step-by-Step Guide for each system. The students\' step-by- step guide is translated to Arabic by Ms. Amina Al-Jabri. Furthermore, she conducted several meetings with Academic and Registration staff members to explain and guide them in using the Online Forms System.
